Applications may be submitted throughout the school year. A $25 application fee is due with each application. Families currently enrolled, past patrons, and church members are given first priority.

State Health Form
The Commonwealth of Virginia School Entrance Health Form is required by Virginia State Law. It must be completed each year by both the child’s parents and physician prior to the child entering school.

Food Allergy Action Plan
If an EPIPEN is needed for a child with a food allergy, this form needs to be completed by both the child’s parents and physician.
